Body Mass Index Explained

BMI (Body Mass Index) is the most widely used weight-screening metric in the world. Doctors, insurers, and public health agencies rely on it — yet it was never designed to measure individual health.

Belgian mathematician Adolphe Quetelet created the formula in the 1830s to describe the "average man" across populations. It became a medical standard in the 1970s and was officially adopted by the WHO in the 1990s. Understanding this origin is key: BMI is a statistical tool, not a diagnostic instrument.

How BMI Is Calculated

The formula is straightforward:

Metric units:

BMI = weight (kg) ÷ height (m)²

Imperial units:

BMI = (weight (lbs) ÷ height (inches)²) × 703

Worked example — 80 kg, 1.75 m:

BMI = 80 ÷ (1.75)² = 80 ÷ 3.0625 = 26.1

A BMI of 26.1 falls in the Overweight category.

The multiplication by 703 in the imperial formula simply converts pounds-per-square-inch into the same scale as kilograms-per-square-meter.

Why Subcategories Matter

The broad labels (Underweight, Normal, Overweight, Obese) are useful for quick reference, but the subcategories reveal important risk differences:

  • Mild vs. severe underweight: A BMI of 17 carries moderate risk; a BMI of 15 signals danger of infection, bone loss, and organ damage.
  • Obese Class I vs. Class III: A BMI of 31 is not equivalent to a BMI of 42. Class III obesity is associated with significantly higher mortality and surgical complications.

Major Limitations of BMI

BMI remains popular because it is cheap, fast, and easy to calculate. But it has well-documented blind spots.

No Distinction Between Muscle and Fat

Muscle weighs roughly 18% more than fat by volume. Two people can share the same BMI while having dramatically different health profiles:

Health risks at a given BMI differ across populations:

  • Asian adults face elevated diabetes and cardiovascular risk at lower BMI values. The WHO recommends BMI 23 as the overweight threshold for Asian populations — two points below the global standard.
  • Pacific Islander populations may show higher BMI at equivalent body fat percentages due to differences in bone density and lean mass.

Fat Distribution Is Ignored
